No, you are not losing your mind. You are losing oestrogen.

You lost the word mid-sentence again, and a small voice asked whether this is how it starts.

It is not. It is your oestrogen, not your mind.

Between 40 and 60 per cent of women report cognitive symptoms during the menopause transition, and in some surveys the memory and concentration complaints run higher than the hot flushes. In one large clinic cohort, memory problems and difficulty concentrating sat near the very top of the symptom list, above the night sweats most people assume define this stage of life. You are in good and crowded company.

 

The reason is oestrogen, and specifically oestradiol. Your brain is full of oestrogen receptors, concentrated in the hippocampus and the prefrontal cortex, the regions that run memory and executive function. In perimenopause oestradiol does not glide down in a neat straight line. It swings, high one week and low the next, before the overall trend settles lower. Your brain, finely tuned to that signal, feels the turbulence. Verbal memory, finding the right word, holding a thread through a meeting, tends to take the brunt of it.

 

Here is what the research is genuinely clear about, and it is reassuring.

For most women this is real, it is hormonal, and normal cognitive function is typically maintained throughout. Only around 11 to 13 per cent show truly measurable impairment, and dementia is rare at this age. This is a transition, not a diagnosis. The fog also rarely travels alone, because lost sleep, raised stress and hot flushes all make it worse, which is part of why it can feel so total in a difficult week and lift in a calmer one.

 

What frustrates me is how many women carry this in silence, quietly frightened, because no one ever told them that the brain is a menopausal organ too. Naming it is half the relief. The other half is addressing the things sitting around it, the sleep, the stress load, the wider hormonal picture, rather than gritting your teeth and white-knuckling through your own forties.

 

Imagine walking into that meeting with the thread held, the word there when you reach for it, because you finally understood what was driving the fog and supported it properly.
